Please fill out for every student coming to the Merge Launchpad Event

Parent/Legal Guardian Information

Waiver

Launchpad requires all students to have a signed waiver form from their Parent/Legal Guardian. They request it to be a paper form. Forms will be given out to students at Merge on Monday March 24, March 31, and April 7. Forms will also be available to be signed at check in on April 11. 

Payment

The cost of the event is $20/student.
Please pay in person on Friday, April 11

Pick Up/Drop Off

Drop Off: 6:30pm (We are leaving at 6:40pm sharp, please arrive with enough time to fill out form if needed)
Pick Up: 9:45pm

Both at Daystar Church

Additional Information

We recommend brining an extra $10-$20 if a student would like to purchase some snacks at Launchpad, although this is optional.

The main point of contact for this event will be our youth leader Bruce Avison, as our Youth Pastor, Liam Briggs, is on a missions trip in Cambodia! Please email bruceavison@hotmail.com if you have any questions regarding the Launchpad Event.
